Fulltime, 32 hours/week. Will cover both Bedford and Nashua locations.

Provides direct peer support and serves as a liaison to the care team for those affected by HIV/AIDS to enhance their decision-making and problem-solving skills, including identification of risk-reduction strategies. Coordinates consumer feedback for the program. Functions as a community- and home-based extension of our program.

Responsibilities:
  1. Coordinates with nurse care managers and provides support for HIV-infected youth (ages 13-24) to develop skills for living with this condition. Provides emotional support through phone calls, home visits, hospital or nursing home visits.
  2. Attends pediatric HIV clinics in Lebanon and Manchester.
  3. Attends appointments with clients as needed. Provides transportation to appointments as needed and able.
  4. Works with the Family Counselor to coordinate and run community-based client/family support groups and assist in follow-up with those identifying needs during the support groups.
  5. Works with the team to identify and develop ways to obtain family feedback about program services and family needs. Assists the team in needs-assessment activities.
  6. Works with the team to find and provide resource information.
  7. Maintains logs of individual in-person and phone contacts.
  8. Participates in planning and implementing recreational and support activities.
  9. Identifies own learning needs and attends trainings to meet them.
  10. Participates in statewide HIV-planning bodies as able.
  11. Performs other duties as required or assigned.
Minimum Qualifications:
  • Graduation from High School or equivalency; some college preferred
  • Strong knowledge of HIV disease and treatments
  • Experience providing support to those affected by the stresses of chronic illness
  • Experience facilitating group support activities
  • Experience in the advocacy role through work experience or training
  • Experience with computer programs such as Microsoft Word and Excel
  • Good oral and written English communication skills
  • Good telephone skills
  • Spanish skills desired 
Required Licensure/Certification Skills:
  • Valid driver’s license and ability to drive own vehicle in Vermont and New Hampshire
